Patient is a 24-year-old female with a long-standing desire to change the appearance of her nose. She felt that her nose was very crooked, and disliked the hump apparent on her profile. She had always been self-conscious of her retracted nostrils, feeling that it made her nose look as though one could easily look inside. She wanted a nose that looked more symmetric, more feminine and elegant, and that complemented her otherwise very fine features.
Rhinoplasty with correction of deviated nasal septum, reduction of dorsal hump, contouring of nasal tip cartilages, and placement of cartilage and fascial grafts to improve the nasal profile and nostril aperatures. She felt that her nasal bones were too wide and irregular, and that her tip lacked definition, and both made her appear a bit unrefined. Patient 23
This patient is a 20 year-old female who had long desired a more feminine nose that appeared to better fit her facial features. She disliked her prominent nasal hump and wanted her nasal tip to appear softer and more elegant. She was bothered by her small chin and was reminded of her long standing dislike of her side profile when she saw photographs of herself. She had dreamed of a smaller nose that appeared natural and a more proportioned chin and jawline that balanced and accentuated her natural facial features.
Rhinoplasty/Septoplasty with placement of both fascial and cartilage grafts, and chin augmentation. A fascial graft harvested from beneath the patient’s scalp was placed at the top of the nose beneath the eyebrows to soften the takeoff between forehead and nose which serves to make the nose appear shorter and more feminine. The large dorsal hump was removed, and nasal tip cartilages were softened and defined through both reduction and suture techniques. Tip projection was supported with cartilage grafts while the lines of the nose (dorsal aesthetic lines) were recreated giving the nose a better flow and very natural appearance. Lastly, the chin was augmented with placement of a large extended anatomic chin implant that provides volume to the chin and jawline imparting balance to the entire face. She was delighted with her surgical result, and her self-esteem was exponentially increased. Photos were taken only 4 weeks after surgery with still significant nasal tip swelling that will diminish greatly over the next year.
Patient 24
Patient is a 22-year-old female who just graduated from college and had always considered herself pretty, but had been long bothered by her nose, especially when she saw herself in photos. Specifically, she felt that her nose was over-projecting, not straight, and wanted like to have the bump taken away. She would like a less bulbous nasal tip, and a nose that appeared more like a model’s.
Rhinoplasty Septoplasty with straightening of the nasal septum, reduction of nasal dorsal hump and narrowing of nasal bones, and contouring of the nasal tip through cartilage reduction and suture techniques. Cartilage grafts were placed to maintain strength and support to the nose to maintain her beautiful result for years. Notice how her dorsal aesthetic lines flow from the top of her nose to its base. This is what makes a nose appear attractive and creates shadows that photograph beautifully.
Patient 25
Patient is an 18-year-old female who for many years had desired a more beautiful and feminine nose. She felt that her nose was crooked and too long, and did not like the dip where her nose starts at her forehead as it tends to make her look harsh. She thinks her nasal tip is too broad and hooks down when she smiles or laughs. She would like a nose that appears softer and straighter and naturally complements her other facial features.
Rhinoplasty with septoplasty with straightening of the nasal septum, mild reduction of nasal hump, and softening of nasal tip cartilages to increase definition and angularity. Overall nasal tip projection was reduced, but in a controlled fashion with cartilage grafts placed to maintain tip support. A fascial graft was placed in the radix (top of nose between eyebrows) to soften the forehead/nasal junction and visually shorten the nasal profile.
Patient 26
Patient is a 20-year old female who had been employed as a runway model for the last few years. She is of German dissent and had desired a softer and more attractive nose since she was in middle-school. As with every patient, I took photographs of the patient’s nose and imaged what I thought I could produce in her best interest, and had her bring in photographs of what the patient thought were beautiful noses. We evaluated what she was looking for and came up with a plan that incorporated what she desired.
Procedure: Rhinoplasty with Septoplasty with placement of a fascial graft to the nasal radix (from the right scalp) to elevate the point where the nose started to make the nose appear shorter and more feminine. The nasal dorsal bump was softened, and the nasal bones were narrowed to produce elegant dorsal aesthetic lines. The nasal tip was narrowed, shortened and rotated through cartilage removal and suture techniques, and the new tip position was strengthened with cartilage grafts from the septum. Lastly, to make the nostrils appear more symmetric, the right nostril was both shortened and flare reduced to complete the nasal surgery. She is elated with her surgical results with greatly enhanced her confidence in her personal life and career.
Patient 27
Patient is a 22-year-old female who is a nurse and had a long-standing desire to have a rhinoplasty. She had never been fond of her profile and felt that her nose was too wide and without definition. She was self-conscious in photos as her nose would turn downward and her nostrils would flare. She desired a prettier nose that appeared more perky and feminine, while simultaneously naturally complementing her facial features.
Rhinoplasty with septoplasty with reduction of dorsal hump, increase of nasal tip projection, and contouring of nasal tip cartilages through suture techniques. A fascial graft was placed at the radix (top of nose) to her profile appear more elegant, and create more appealing dorsal aesthetic lines. She also underwent a total of 9 cc of autogenous fat transfer to her face, including periorbital, midface, and lips.
Patient 28
Patient is a 23-year-old Hispanic female who had long desired a perkier and more feminine appearing nose. She specifically did not like her large dorsal hump and poorly projected nasal tip that drooped when she smiled. She desired a more refined and attractive appearing nose that better complemented her facial features, and hopefully augments her self-esteem.
Procedure: This patient is a classic example of someone with a classic hispanic nose. She had a dorsal hump and lacked nasal tip support that allowed the tip to droop upon smiling. She underwent a rhinoplasty septoplasty and reduction of boney dorsal hump and narrowing of the nasal bones with recontouring to create attractive dorsal aesthetic lines. She had placement of a cartilage strut connected to the now midline nasal septum to support the nasal tip and allow contouring of the nasal tip through suture techniques, and rotation to make the nose appear perky. The caudal septum was deepened to sharpen the angle between the lip and nose which served to make the upper lip look longer and more aesthetic, bringing balance the entire face. The patient was extremely happy with her results. She felt prettier, more confident, and others had difficulty exactly deciphering what was done.
Patient 29
Patient is a 16-year-old female with a desire to have a more attractive and feminine appearing nose. Specifically, she did not like her dorsal bump, her perceived wide nasal bones, and the lack of projection of her nasal tip that went down when she smiled. She had done extensive research on the internet and social media regarding the kind nose she desired and was seen in the office with her mother on two separate occasions to confirm her motivation and the result she desired.

Patient 30
Patient is a 24-year-old nurse who underwent a previous rhinoplasty by another surgeon who despite his confidence and assurances, was not successful in making the patient happy. She specifically did not like her flat and masculine appearing nose that lacked feminine tip projection and did not appear “perky” as she was promised. She did not like her profile as she felt that her nose is dominated by a bump and a wide and downturned nasal tip that appears to droop more when she smiled. She wanted a more feminine nose that had more delicate contours with a smaller and upturned nasal tip that better complemented her facial features.
Procedure: Secondary Rhino/Septoplasty with softening of dorsal hump, narrowing of nasal bones and recontouring of nasal tip cartilages through suture techniques. It was necessary to utilize an autologous cartilage graft (cadaveric costal cartilage graft) to provide support in order to increase nasal tip projection and maintain long term tip support. The patient also underwent autogenous fat transfer to the lower eyes and periorbital region to soften longstanding hollowing and pigmentation. She was absolutely delighted with her results and that is truly the best part of my job!
Patient 31
Patient is a 19-year-old female who desired a more elegant nose that would better complement her facial features. Specifically, she felt that her nose appeared masculine and wanted to address the dorsal hump and over-projecting nasal tip that drooped when she smiled. She disliked her nasal tip and was looking for better definition, but wanted her nose to appear natural and unoperated on. Her desires were communicated through pre-operative imaging and a portfolio of noses from magazines that she deemed attractive.
Septorhinoplasty with reduction of dorsal hump, narrowing of nasal bones, and contouring of the nasal tip through cartilage reduction and suture techniques. A small tip graft was placed to improve tip definition. She is 6 months post-surgery with appropriate swelling in the supra-tip region, and can expect significantly improved definition of the nasal tip over the next six months. She also underwent autogenous fat transfer to the lips for a total of 5cc, with permanently improved lip volume and contour that beautifully complements her facial features.
Patient 32
Patient is a 29-year-old female who desired a perkier and more feminine appearing nose. She did not like the prominent dorsal hump on her profile and wanted to change her nasal tip that went down when she smiled. She wanted a softer appearing nose that better complemented her facial features while appearing natural and maintains her ethnicity.
Rhinoplasty with Septoplasty with reduction of bony hump, narrowing of nasal bones and increase of nasal tip projection with placement of internal cartilage strut. The nasal tip cartilages were both contoured and repositioned to a perkier position. A fascial graft was placed at her radix (the top of the nasal bridge at the forehead) to soften the transition of the forehead to nose which makes the nose look more feminine and elegant. She loves her new nose at only 2 months post-op.

Patient is a 16-year-old female with a long-standing desire to have a more attractive nose and stronger, more balanced profile. She was self-conscious of her nose, and this combined with the weakness of her chin had made her shy. She wanted to have a prettier and feminine nose that better balanced and complemented her other features.
Rhinoplasty with Septoplasty with reduction of nasal bump and refinement of nasal tip utilizing reduction and suture techniques. A fascial graft was placed at her radix (area at top of nose beneath eyebrows) to soften the transition between her forehead and nose giving the illusion of actually shortening the entire nasal profile. Patient 35
Patient is a 27-year-old female with a long-standing desire to straighten and cosmetically enhance her nose. She has felt for years that her nose is both crooked and over-projecting with lack of feminine definition of the nasal tip that detracts from her otherwise attractive facial features. She would like a smaller, more elegant and feminine appearing nose.
Septorhinoplasty with straightening of nasal septal and bony deviations, contouring of nasal bones to enhance the dorsal aesthetic lines, reduction of nasal dorsum and tip over-projection with stabilization through the utilization of cartilage grafts. The nasal tip was also reduced and contoured through suture techniques, with fine-tuning of the aperature of nostrils. She is two months post-rhinoplasty and ecstatic with her results, with future evolution of her results up to a year anticipated.

Patient is a 19 year-old-hispanic female who has long desired a prettier, more proportioned nose that better complements her facial features. She does not like the width of her nose along with her bulbous tip and very prominent nostrils. She has always been self-conscious of having her picture taken, especially from the side, as she has hated the bump and how her nose turns down when she smiles. She has always dreamed of an elegant nose with a delicate tip that looks beautiful and natural.

She was bothered by her dorsal hump and over-projecting nasal tip that had made her very self-conscious about having her photo taken, especially her profile. She hoped for her nose to appear smaller, perkier, and straighter.

Fat was transferred to her periorbital region below her eyes to soften the hollowing and hyperpigmentation of this region. She is delighted with her results; photos were taken approximately 3 months after surgery.

Patient is a 21-year-old female who is Kurdish ethnicity who wanted to have her nose aesthetically enhanced while maintaining her exotic features and identity. She specifically did not like her masculine appearing profile, and heavy wide appearing nasal bones and nasal tip that overpowered her other features. She wanted a softer and more feminine appearing nose that better complemented her facial features.
Procedure: Rhinoplasty with reduction of dorsal hump, lowering of nasal radix to make the nose appear that it starts lower making her nose appear shorter and more feminine. She also underwent narrowing of her nasal bones to create pleasing dorsal aesthetic lines, and her nasal tip was reduced through cartilage reduction and suture techniques. Her nostrils were both shortened and narrowed to make the nasal tip width more proportioned with the rest of her facial features. She was ecstatic with the results of her rhinoplasty as was her entire family.
